Form for staff to report LOST or STOLEN Access Control Badge or Key(s). Before completing this form:
(1) Contact the local police department where the key(s) were lost/stolen. You will need to obtain a service or tag number for this call;
(2) Contact the Safety & Security Department at (562) 602-6024 and verbally report lost/stolen key(s).
Safety Incident Report form is intended to report and document school related incidents on or outside of campus. Incident reports may include criminal activity, threats to staff and students, theft/loss, bulling, fraud, disturbances, vandalism, and other incidents requiring documentation. Please complete and submit the form as soon as possible.
Video Request Form is for administrators investigating school incidents for the purpose of fact finding and the confirmation of individuals involved. Incidents must be connected to school discipline, disruptions, injuries, and or incidents related to school activities.
