Safety Incident Report form is intended to report and document school related incidents on or outside of campus. Incident reports may include criminal activity, threats to staff and students, theft/loss, bulling, fraud, disturbances, vandalism, and other incidents requiring documentation. Please complete and submit the form as soon as possible.

(1) Contact the local police department where the key(s) were lost/stolen. You will need to obtain a service or tag number for this call;
