Video Request Form is for administrators investigating school incidents for the purpose of fact finding and the confirmation of individuals involved. Incidents must be connected to school discipline, disruptions, injuries, and or incidents related to school activities.

How we process your request
After you submit your request the team will:
- Review the request
- Contact you for further information (if necessary)
- Expected timeline
- Send a closeout notification
Before you submit a ticket, you will need to:
- Describe the issue or incident in detail
- Time, date, and area/location the incident occurred
