Overview
Safety Incident Report form is intended to report and document school related incidents on or outside of campus. Incident reports may include criminal activity, threats to staff and students, theft/loss, bulling, fraud, disturbances, vandalism, and other incidents requiring documentation. Please complete and submit the form as soon as possible.
Who can submit this request?
Any employee of PUSD.
Requesting Service
The District’s Safety Department will notify the reporting party of the next steps and agency collaborations.
