Parents are now able to submit the TDAP Immunization document via ParentVUE. Follow steps below:
1. Log into ParentVUE
2. Click ParentVUE, get to the screen where you can see your child(ren)
3. Select your middle school student:

4. Go to Student Info tab
5. Click on the Edit Information button

6. Scroll down to find the Upload Documents area. Drag & Drop file ot Click in box to upload.

7. Upload document and click Save Changes

Upload is confirmed

8. The school site's health office technician will process the request.
