Enrollment Benefits Application

Overview

If needed, parents can complete an enrollment benefits application via ParentVUE.

Step-by-Step Instructions

  1. Log into ParentVUE, Select Online Registration Tab and Select the Universal Benefits Form

Introduction Tab

  • Review welcome, select Continue
  • Identify the Student(s), select Save And Continue
  • Type your name to capture Signature, select Save And Continue

Family Tab

  • Review Home Address and select Save And Continue
  • Review Mail Address and select Save And Continue

Parent/GuardianTab

  • Review Demographic information and select Save And Continue
  • Complete Universal Benefits Form
    • Select Household size
    • Check the services that apply
    • Date
    • Apply Signature to sign
    • Select Save And Continue
  • Find your name and select Edit then Save And Continue

Emergency Tab

  • For each emergency contact listed select Edit then Save And Continue
  • Make sure there is a Complete Status for all 
  • Select Save And Continue

Student Tab

  • For each student listed select Edit then Save And Continue
  • Make sure there is a Complete Status for all 
  • Select Save And Continue

Review & Submit Tab

  • Select Review
  • Scroll down, Universal benefits information is correct then check box
  • Select Submit
  • Confirm submission then select Ok
